I’ve just been enrolled into Kaiser Permanente’s “Complex Case Management Program.” Who knew they had one?!………and why did it take them so long to figure out I needed it?!
It’s a special program that helps members with more than one ongoing health condition receive high quality care. You are assigned an RN Case Manager to work with your Doctor not only to help you successfully manage your health conditions but to develop a care plan for you that meets your medical, emotional and social needs while matching you with the right Kaiser Permanente services & community services if you need them.
So far what I am getting out of this is a Nutritionist & a “Healthy Living” Coach. The Coach is a free service and the Nutritionist is a co-pay. I am expecting my first call from my Coach Kimberly from California @ 7am this morning. It will be interesting to hear what she has to say.
I have a few goals I am setting. 1) to eat healthier & stop compulsive eating
I need to get some weight off. 1) I weigh too much 2) Any kind of weight I can get off will help my lung function 3) I want to treat my body with more respect by putting good things into it
Goal 2) to start moderate exercise
Having had two hospitalizations in one year and spending so much time down/sedentary/recuperating I have so much muscle mass and stamina. I am realizing that if I do not WORK to get it back that it will never come. I’m not going to be a hard ass about it and go out and walk everyday, but my goal is 3-5x a week starting off walking at a slow pace for 30-45 min and go from there. This is all I can do right now and I am really tired when I am done, so I am giving myself grace and patience to let my body take the time to heal itself.
Also, I just have to add that I’ve had another day without a migraine!!! This is unbelievable (but I am believing it)!!! It is so great to not have to take my reversal meds which make me feel wiped out. Don’t get me wrong, I’m thankful for the DHE & Zomig because they rescue me in my times of trouble, but no trouble @ all is better news to me!!!!!!!!!

Allergies, Migraines, Bipolar, Premenstrual Dysphoric Disorder, Kidney Stones, History of Pneumonia & I hate to say it but it’s pretty obvious, Obesity.
I am really happy about being enrolled in this program and feel like the troops are gathering to battle for me. I can’t take this journey alone. I’ve tried it so many times and hit a road block which results in no lasting change. Now I have all of these little warriors surrounding me so that when I do fall they can help pick me back up and encourage me to keep going.
Coach Kim is going to call in about 45 min.

Seems like all of us need a “TEAM” sometimes. Thank you Kaiser for whatever you can do to help our D.D. to a better quality of life. We want her to be all she can be! ❤
